Appointment lifecycle metrics (event-in-period semantics)
Counters reflect events whose state transitioned during [from, to). created uses appointments.created_at; scheduled uses start_time; completed/cancelled use updated_at on rows in that status. by_source_channel attributes appointments via the linked conversation; appointments with no conversation appear as ‘other’ (only when no channel filter is supplied).
